Role Description

We are seeking the immediate engagement of a full-time, on-site Patient Care Coach/Educator to join our Clients team in Kuwait. As a Patient Care Coach/Educator, you will be responsible for providing patients with coaching and education on how to manage their health and well-being, as well as helping them to navigate the healthcare system. Key responsibilities include developing personalized care plans for patients, facilitating care transitions, providing patient and family education, and monitoring patient progress. You will work collaboratively with the patient’s care team to help achieve optimal health outcomes.

• The Patient care Coach/educator provides compassionate and empathetic guidance to help patients manage the impact of their condition and to receive their treatment effectively.
• The educator is a qualified healthcare professional who complements the education and care provided by the patient’s own healthcare team.
• The role involves regular patient contact, in person and by telephone, and is responsible for building rapport and helping to address their patients’ personal challenges and needs.
• Facilitate successful communication between patients, physicians, Multidisciplinary teams, and third-party payers that leads to shared motivation to achieve treatment goals.
• Ensure continuous patient education, motivation, consultation, and follow-up.

Requirements

• Must have a bachelor’s degree in Nursing or Pharmacy
• Excellent written and oral communication skills in Arabic and English are a must.
• Must have a Driving license.
